Scheduling an Appointment
All appointments are scheduled by our Traverse City office, please call 231-935-9275.

Our patients are referred to us by physicians or other health care professionals, and direct self referrals are accepted for adult patients. A copy of the sleep specialist's evaluation will be sent to your referring health care provider with recommendations for treatment. Relevant medical records should be provided so that we can properly evaluate your problem. A list or your current medications is especially important.

Our office hours are Monday through Friday 8:30 a.m. to 5 p.m. If you are unable to keep your appointment please give us 24 hours notice so that we may schedule other patients who need our help.

Clinics
Our practitioners have clinics in Traverse City, Cadillac, Manistee, Gaylord, and Sault St. Marie. Consultations are preformed at these sites for the convenience of our patients. Click here for clinic locations.

Questionnaire
You will receive a sleep questionnaire that is to be filled out by the patient as well as by a bed partner, or family member, roommate etc. who may be familiar with the patient's sleep habits. Bed partners are encouraged to come to the initial consultation with the sleep specialist to help with the history, since most patients are not able to describe what they do while asleep. Bed partners are not required at the time of the overnight sleep study and there is limited room for them to stay.

The Initial Visit
The initial appointment will take approximately one hour. If you have not received or completed your sleep questionnaire you should arrive ten to fifteen minutes early or you may download it now by clicking on this link. You may be shown a video with education information about a sleep study and your suspected sleep disorder.

On this initial visit the practitioner will decide if a sleep study is necessary. Some disorders such as sleep apnea or narcolepsy require an overnight sleep study to diagnose and treat, whereas other disorders such as restless leg syndrome, circadian rhythm disorder, or insomnia may require an evaluation by the doctor resulting in treatment, but usually no overnight study is required.

Confidentiality
For your protection, no records will be released from this office without a written and signed authorization.

Office Co-pays
We will collect your office co-pay amounts on the day of service. This should be the same co-pay you would expect to pay at your primary care office.

Insurance
Sleep disorders are covered by insurance policies the same as other medical disorders. We participate with Mercy Preferred Choice, Medicare, Blue Cross Blue Shield, Community Blue PPO, Priority Health, Blue Care Network and PPOM. If you have another medical insurance, we are happy to accept your insurance company's approved amount as payment for services. You will be responsible for any co-payment, deductible amounts, and non-covered services as determined by your insurance company. We submit claims to your insurance company as a service to our patients. If you require a payment program this may be discussed with the office in advance of your first appointment.
